Appointment Information
OAM has partnered with Phreesia to offer you the ability to complete the pre-registration process remotely prior to your appointment. You will receive a text message and/or email that contains a link to privately pre-register a few days before your visit, making your arrival at the office quick and simple. If you did not receive the registration link, please call our office at 616-459-7101.
Upon your arrival, please enter the building to check in for your appointment. We are limiting the number of patients that are able to be present in our waiting rooms to maintain six feet of distance. You may be asked to return to your car to wait for your appointment if we have reached our capacity.
At this time, we are limiting visitors at all locations. Two visitors may accompany an adult patient to their appointment and both parents may accompany a minor patient to their appointment. We currently ask that any Vendors please stop by the concierge desk in the main entryway at the Leffingwell and Leonard Street Offices and at the main registration desks at our other locations to drop off information.
